Can you please describe the exact process you are using to post the Inventory Adjustment journal? (For example, manual posting vs batch job, or if multiple tabs are open.)
Have you checked the user log entries to see if there were two sessions open for the same user around the time the journal was posted? Sometimes, a user could be logged in from another session (remote desktop, another browser) and that can cause confusion.
It might also be a good idea to ask the user to change their password and monitor if the issue happens again — just to rule out any potential session conflicts or shared credential usage.
